Maintaining a clear pathway at all times to aide in fall
prevention. Walking with patient as much brushing teeth.
Assisting with dressing, complete or partial help. Assist in all incontinence care, from complete care, to stand by assistance. Changing of diapers or pull ups and cleaning patient. Monitoring of perineal area for skin break down.
Applying lotion as needed/ wanted. Brushing and combing of hair
as needed, or helping patient into and out of wheelchair. Teach/reenforce teaching of assistive devices. Encouraging patient to notify caregiver with any needs. Including pain assessment. Ability to notify family of any changes or new findings.
